New Location Onboarding

This form is designed to gather all the necessary information required to create your new location.

STEP 1

Log into your Merchant Portal and do the following:

  1. Create your new location (Settings -> Locations -> Add Location) with all the necessary details. (Click here for instructions)
  2. Create a new Tablet User by going to "Order Tablet User" then "Add New User" and associate this new user with this new location. 
Note: You may leave this location as inactive at this time.

STEP 2

Once you've completed creating your location in the Merchant Portal, submit the form below.

Please note that all the information required in this form is mandatory. We kindly request that you fill out the form in its entirety before submitting it. This will enable us to accurately assess your needs and provide you with the best possible service.

By submitting this form, you authorize Smooth Commerce to charge a one-time setup fee of $150 to set up your new location. This fee covers the cost of processing your application and setting up your new location.

Please note that this fee is non-refundable and will be charged on your next round of billing. If you do not wish to proceed with the setup of your new location, please do not submit the form.
